چکیده

SUMMARY The instantaneous response of the left ventricle to loss or recoverv of effective atrial contraction was studied in 12 patients with valvular aortic stenosis by producing A-V dissociation by electrical pacing of the ventricles so that some ventricular contractions were preceded by an effective atrial contraction while others were not. A conventional hydraulic formula relatinig size of the aortic valve orifice, pressure gradient, and flow was used to estimate beat to beat stroke volume. A properly timed atrial contraction augmented left ventricuilar performance in all patients, increasing stroke volume index from 14 to 55% (mean, 27%) anid stroke work from 17 to 85% (mean, 453%).

Left atrial booster function in valvular heart disease.

This study was designed to assess atrial booster pump action in valvular heart disease and to dissect booster pump from reservoir-conduit functions. In five patients with aortic stenosis and six with mitral stenosis, sequential atrioventricular (A-V) pacing was instituted during the course of diagnostic cardiac catheterization. Continuous recording of valvular gradient allowed estimation of flo...

Impaired rate of left ventricular filling in idiopathic hypertrophic subaortic stenosis and valvular aortic stenosis.

In order to determine whether there is any interference with left atrial emptying or left ventricular filling in idiopathic hypertrophic subaortic stenosis (IHSS) and aortic stenosis, the fall in pressure (y descent) of the left atrial v wave following the opening of the mitral valve was analyzed in 27 patients with IHSS and in 22 patients with valvular aortic stenosis, and the results were com...

Systolic time intervals in valvular aortic stenosis and idiopathic hypertrophic subaortic stenosis.

Corrected systolic time intervals of the left ventricle: the interval between the onset of the Q wave and the first heart sound (Q-MI), isovolumic contraction time (IVCT), and left ventricular ejection time (LVET) have been measured by noninvasive techniques in 26 patients with valvular aortic stenosis and in 25 patients with idiopathic hypertrophic subaortic stenosis. The results were compared...

A Hemodynamic Technic for the Detection of Hypertrophic Subaortic Stenosis

IT IS NOW well appreciated that following a successful valvulotomy for valvular pulmonic stenosis, muscular hypertrophy in the right ventricular outflow tract may persist and be responsible for a significant resistance to right ventricular ejection.'-3 Similar muscular obstruction to the left ventricular outflow has also been noted in patients following surgical relief of discrete valvular or s...

ABC of clinical electrocardiography. Conditions affecting the left side of the heart.

Many cardiac and systemic illnesses can affect the left side of the heart. After a careful history and examination, electrocardiography and chest radiography are first line investigations. Electrocardiography can provide supportive evidence for conditions such as aortic stenosis, hypertension, and mitral stenosis. Recognition of the associated electrocardiographic abnormalities is important as ...
